This chair was made by Bassett it had the number 1425-02 on the bottom which may be a model number., the chair has a loose back pillow, loose seat cushion, and some material on the bottom and arms, the cover is made to tie under the bottom and the arms are covered and attached with Velcro, the pillow and cushion are covered seperately

Installation Notes: When installing this set of covers, The cushion cover will have some zipper showing on the sides, this is for easier installation, you will have to bend the cushion a little to get the cushion cover on, the arm covers come as right and left, try both ways for best fit.
